If NO go to question 3.

If YES, is the official vehicle registration address listed as 4141 N Henderson Rd? ____ YES      ____ NO

If YES, stop. Application is complete. Submit application and $100 check to the office.

If NO, you are not eligible to register your car to park at Hyde Park. See Hyde Park rules and regulations.

If NO, you are not eligible to register you car to park at Hyde Park - See Hyde park rules and regulations.

If NO, you are not eligible to register you car to park at Hyde Park - See Hyde park rules and regulations.

If YES, submit your application and $100 check to the office. However, a copy of your lease agreement must be on file in the office before you will receive a decal.
